    Special Orders form for website

    Hi, I have created my site and need a little help.  I have found the create a form where information can be collected from the customers.  I need to be able to add more fields to this.  I need name, email, product, quantity, description,date needed.  I may need more but I can't think right now.  Is there any place that I can specially design the form that I need?  I am not very computer language intelligent so something that speaks plain english will be helpful.

    I am not an expert on the forms, but I am aware there are some limitations with them.  There are also some third-party forms services some of us have used.  Allforms and Jotform come to mind.
